Chapter 1: Foundations of Information Security

1.1 What Is Information Security?

Information security is the practice of protecting information and the systems that store, process, and transmit it from unauthorized access, use, disclosure, disruption, modification, or destruction. As organizations have become profoundly dependent on digital infrastructure, the discipline has evolved from a narrow technical concern into a strategic management imperative that touches every part of an enterprise.

Information Security — The protection of information and information systems from unauthorized access, use, disclosure, disruption, modification, or destruction in order to provide confidentiality, integrity, and availability.

The field draws on computer science, management, law, psychology, and engineering. A cybersecurity professional must understand not only firewalls and encryption but also human behaviour, organizational governance, and regulatory environments.

1.2 The CIA Triad

The most widely cited model in information security is the CIA triad: Confidentiality, Integrity, and Availability. Every security control, policy, and architecture decision can be evaluated against these three properties.

Confidentiality — Ensuring that information is accessible only to those authorized to access it. Breaches of confidentiality include unauthorized data exfiltration, eavesdropping, and shoulder surfing.
Integrity — Ensuring that information and processing methods are accurate and complete, and that data has not been altered in an unauthorized manner. Integrity encompasses both data integrity (the content itself) and system integrity (correct system operation).
Availability — Ensuring that authorized users have reliable and timely access to information and resources when needed. Denial-of-service attacks, hardware failures, and natural disasters all threaten availability.

Consider a hospital’s electronic health records system. Confidentiality means only authorized physicians and nurses can view patient files. Integrity means a patient’s blood type cannot be silently altered. Availability means clinicians can access records during an emergency at 3 a.m. A failure in any one dimension can endanger lives.

1.3 Beyond the CIA Triad: The Parkerian Hexad

Donn Parker proposed an expanded model with six properties, arguing that CIA alone cannot capture the full spectrum of security concerns.

Parkerian Hexad — A six-element framework for information security comprising confidentiality, integrity, availability, possession (or control), authenticity, and utility.

The three additional elements are:

  • Possession (Control): Having physical custody or control over information. A stolen encrypted laptop compromises possession even if the attacker cannot read the data.
  • Authenticity: Assurance that information, transactions, or communications are genuine and originate from the claimed source.
  • Utility: Information must be in a format that is useful. Encrypted data for which the decryption key has been lost satisfies confidentiality and integrity, but has zero utility.

1.4 The Threat Landscape

A threat is any potential cause of an unwanted event that may result in harm to a system or organization. Threats exist in a landscape shaped by the motives, capabilities, and resources of adversaries alongside the vulnerabilities present in systems.

Vulnerability — A weakness in a system, process, or control that could be exploited by a threat to cause harm.
Risk — The potential for loss or harm arising from the interaction of a threat with a vulnerability. Risk is commonly expressed as:

Attack Vectors and Categories

Attack vectors include network-based attacks (man-in-the-middle, packet sniffing), application-layer attacks (SQL injection, cross-site scripting), social engineering (phishing), physical attacks (USB drops, tailgating), and supply chain compromises.

The MITRE ATT&CK framework catalogues adversary tactics, techniques, and procedures (TTPs) across the attack lifecycle, from initial access through lateral movement to exfiltration. It serves as a common language for defenders to describe and detect threats.

1.5 Defence in Depth

No single control can stop every attack. Defence in depth (also called layered security) employs multiple overlapping controls so that if one layer fails, subsequent layers continue to protect the asset. A typical layered architecture includes:

  1. Physical security — locked server rooms, surveillance cameras
  2. Network security — firewalls, intrusion detection/prevention systems, network segmentation
  3. Host security — endpoint detection and response (EDR), host-based firewalls, patch management
  4. Application security — secure coding practices, web application firewalls, input validation
  5. Data security — encryption at rest and in transit, data loss prevention (DLP)
  6. Administrative controls — policies, training, background checks

This concept maps directly to military doctrine: concentric rings of fortification ensure that breaching one wall does not mean the citadel has fallen.


Chapter 2: Identification, Authentication, and Authorization

2.1 The IAA Process

When a user attempts to access a system, three distinct steps occur in sequence: identification, authentication, and authorization. Though often conflated, each serves a different purpose.

Identification — The process of claiming an identity (e.g., entering a username). It answers the question: Who are you?
Authentication — The process of verifying that the claimed identity is genuine (e.g., entering a correct password). It answers: Can you prove it?
Authorization — The process of determining what actions or resources the authenticated identity is permitted to access. It answers: What are you allowed to do?

A library analogy illustrates the distinction: showing your library card is identification; the librarian checking the card photo against your face is authentication; the card’s borrowing privileges (student vs faculty) determine authorization.

2.2 Authentication Factors

Authentication mechanisms are classified into three canonical factors, often described as something you know, something you have, and something you are.

Emerging literature recognizes additional factors: somewhere you are (geolocation), something you do (behavioural biometrics such as keystroke dynamics and gait analysis), and someone you know (social authentication).

2.3 Passwords: The Persistent Problem

Despite decades of research into alternatives, passwords remain the dominant authentication mechanism. Their persistence reflects low implementation cost and universal user familiarity — but also creates enormous security challenges.

Password Attacks

  • Brute force: Systematically trying every possible combination. Feasibility depends on password length and character set. A six-character lowercase password has \( 26^6 \approx 3.1 \times 10^8 \) possibilities — trivial for modern hardware.
  • Dictionary attacks: Trying words from a dictionary and common password lists. The 2009 RockYou breach exposed 32 million plaintext passwords; “123456” appeared over 290,000 times.
  • Credential stuffing: Using username-password pairs leaked from one breach to log into other services. This succeeds because users reuse passwords across sites at a rate estimated between 60% and 73%.
  • Rainbow tables: Precomputed hash-to-password lookup tables that dramatically speed up offline cracking. Defeated by salted hashing.
  • Phishing: Tricking users into entering credentials on a fraudulent site. Unlike the above attacks, phishing targets the human rather than the cryptographic mechanism.

Password Storage

Responsible systems never store passwords in plaintext. The standard approach is to store a salted hash: a random value (the salt) is concatenated with the password before hashing. The salt is stored alongside the hash. Modern password-hashing algorithms — bcrypt, scrypt, and Argon2 — are deliberately slow (computationally expensive) to impede brute-force attacks, a technique called key stretching.

The Human Dimension of Passwords

People infuse passwords with autobiographical meaning — names of children, anniversary dates, favourite songs. This emotional investment makes passwords more memorable but also more guessable, particularly through open-source intelligence (OSINT) gleaned from social media. Research has shown that people’s password choices often reflect deeply personal themes: cherished memories, aspirational identities, or private jokes. This intertwining of identity and authentication creates a paradox: the passwords easiest to remember are often the easiest to guess.

Password fatigue — the cognitive burden of maintaining dozens of unique, complex passwords — leads predictably to password reuse and simplification. Password managers address this by generating and storing unique, high-entropy passwords for each service, requiring the user to remember only a single master password.

2.4 Biometric Authentication

Biometric systems measure physiological or behavioural characteristics to verify identity. Every biometric system involves enrollment (capturing a reference template), storage (keeping templates securely), and matching (comparing a live sample against the stored template).

Key performance metrics include:

  • False Acceptance Rate (FAR): The probability that the system incorrectly accepts an unauthorized person. Also called Type II error.
  • False Rejection Rate (FRR): The probability that the system incorrectly rejects an authorized person. Also called Type I error.
  • Crossover Error Rate (CER): The point at which FAR equals FRR. A lower CER indicates a more accurate system.

Unlike passwords, biometric characteristics cannot be changed if compromised. If an attacker obtains your fingerprint template, you cannot simply “reset” your fingerprint. This irreversibility makes template protection critical — biometric data should be stored as encrypted mathematical representations (feature vectors), never as raw images.

2.5 Multi-Factor Authentication (MFA)

Multi-Factor Authentication (MFA) — An authentication scheme that requires two or more distinct factors from different categories (knowledge, possession, inherence). Using two passwords is not MFA because both factors are from the same category.

MFA dramatically reduces the risk of account compromise. Microsoft has reported that MFA blocks over 99.9% of automated account compromise attacks. Even if an attacker obtains a password through phishing, they cannot access the account without the second factor.

Common MFA implementations include:

  • SMS one-time passwords (OTP): Convenient but vulnerable to SIM-swapping attacks, where an attacker convinces a mobile carrier to transfer the victim’s phone number.
  • Authenticator apps (TOTP): Time-based one-time passwords generated by apps like Google Authenticator or Microsoft Authenticator. More secure than SMS because they do not traverse the cellular network.
  • Hardware security keys (FIDO2/WebAuthn): Physical devices such as YubiKeys that use public-key cryptography. Resistant to phishing because the key verifies the requesting domain cryptographically.
  • Push notifications: The user approves a login attempt via a push notification to a registered device. Vulnerable to “MFA fatigue” attacks, where an attacker repeatedly triggers notifications hoping the user approves one to stop the bombardment — a technique used in the 2022 Uber breach.

Chapter 3: Access Control Models and Mechanisms

3.1 Principles of Access Control

Access control governs who (subjects) can do what (actions) to which resources (objects) within a system. It is the enforcement mechanism for authorization decisions. Several foundational principles guide access control design.

Principle of Least Privilege — Every subject should be granted only the minimum permissions necessary to perform its assigned tasks, and no more. This limits the potential damage from accidents, errors, or unauthorized actions.
Separation of Duties — Critical tasks should be divided among multiple individuals so that no single person has sufficient access to commit fraud or cause harm unilaterally. For example, the person who approves a payment should not be the same person who initiates it.
Need-to-Know — Access to information should be restricted to individuals who require it to perform their duties, even if their security clearance or role would otherwise permit broader access.

These principles work in concert. A financial controller may have a high-level security clearance but should still only access the specific accounts relevant to their current project (need-to-know), should not be able to both create and approve purchase orders (separation of duties), and their system account should lack administrative privileges on the email server (least privilege).

3.2 Access Control Models

Different organizations and systems require different approaches to access control. The four primary models represent different philosophies about who decides access permissions and how those decisions are structured.

Discretionary Access Control (DAC)

Discretionary Access Control (DAC) — An access control model in which the owner of a resource determines who may access it and what permissions they receive. Access decisions are at the discretion of the resource owner.

DAC is the model used in most desktop operating systems. When you create a file on your computer, you decide who can read or edit it. The Unix file permission system (owner/group/others with read/write/execute) is a classic DAC implementation.

Strengths: Flexible, intuitive, easy for users to manage. Weaknesses: Owners may make poor access decisions; difficult to enforce organization-wide policies; vulnerable to Trojan horse attacks (a malicious program running with the user’s privileges can access everything the user can).

Mandatory Access Control (MAC)

Mandatory Access Control (MAC) — An access control model in which a central authority assigns security labels to subjects and objects, and the system enforces access rules that no individual user can override. Access decisions are mandatory, not discretionary.

MAC systems assign classification labels (e.g., Unclassified, Confidential, Secret, Top Secret) to data and clearance levels to users. The Bell-LaPadula model formalizes confidentiality rules: “no read up” (a subject cannot read objects at a higher classification) and “no write down” (a subject cannot write to objects at a lower classification, preventing information leakage). The Biba model addresses integrity with inverse rules: “no read down” and “no write up.”

MAC is used in military and intelligence environments — the Government of Canada’s Protected/Classified information system follows this pattern. SELinux (Security-Enhanced Linux) provides a practical implementation of MAC in general-purpose computing.

Role-Based Access Control (RBAC)

Role-Based Access Control (RBAC) — An access control model in which permissions are assigned to roles rather than to individual users. Users are then assigned to roles based on their job functions.

RBAC maps naturally to organizational structure. Rather than assigning individual permissions to each of 500 employees, an administrator creates roles (e.g., “Accounts Payable Clerk,” “HR Manager,” “IT Administrator”) with predefined permission sets, then assigns employees to appropriate roles. When an employee changes jobs, their role assignment changes — not hundreds of individual permissions.

RBAC supports the principle of least privilege through role engineering: carefully designing roles to include only the permissions necessary for each job function. It also facilitates auditing because reviewers can examine role definitions rather than individual user permissions.

Attribute-Based Access Control (ABAC)

Attribute-Based Access Control (ABAC) — An access control model in which access decisions are based on attributes of the subject (e.g., department, clearance level), the object (e.g., classification, sensitivity), the action (e.g., read, write, delete), and the environment (e.g., time of day, network location).

ABAC provides the most granular and context-aware access control. A policy might state: “Physicians in the Emergency Department may access patient records during their shift hours from hospital-network IP addresses.” This single rule encodes subject attributes (role: physician, department: emergency), object attributes (type: patient record), and environmental attributes (time: shift hours, location: hospital network).

3.3 Access Control Implementation

Access Control Lists (ACLs)

An access control list is a table that defines, for each object, which subjects have which permissions. ACLs are object-centric: they are “attached” to the resource. A file’s ACL might specify that Alice has read/write access, Bob has read-only access, and the Finance group has read access.

Capability Lists

A capability list is the inverse perspective: for each subject, a list of objects they can access and the permitted operations. Capability lists are subject-centric. They answer the question “What can this user access?” rather than “Who can access this resource?”

Physical Access Controls

Access control extends beyond digital systems. Physical security controls include:

  • Mantrap/airlock: A small room with two interlocking doors; the first must close before the second opens, preventing tailgating.
  • Proximity cards and smart badges: RFID or NFC-based credentials that log entry and exit times.
  • Biometric door locks: Fingerprint or iris scanners at sensitive entry points.
  • Security guards: Human judgment for anomaly detection that automated systems may miss.
  • CCTV and video analytics: Continuous monitoring with increasing use of AI for detecting unusual behaviour.

The 2013 Target breach illustrates the importance of segmenting access: attackers gained initial access through a third-party HVAC vendor’s network credentials, then moved laterally to the payment card processing environment. Proper network segmentation and third-party access controls could have contained the breach to the HVAC management system.


Chapter 4: Auditing, Accountability, and Monitoring

4.1 The Role of Auditing in Cybersecurity

Auditing provides the evidentiary foundation for accountability. Without reliable records of who did what and when, organizations cannot detect breaches, investigate incidents, prove compliance, or hold individuals responsible for their actions.

Accountability — The principle that individuals are held responsible for their actions within a system. Accountability requires identification (knowing who acted), authentication (verifying identity), and auditing (recording the action).
Audit Trail — A chronological record of system activities that provides documentary evidence of the sequence of activities affecting a specific operation, procedure, or event. Audit trails enable reconstruction of events after the fact.

4.2 Logging and Log Management

Logs are the raw material of auditing. Operating systems, applications, network devices, databases, and security tools all generate logs that record events such as login attempts, file accesses, configuration changes, and error conditions.

Effective log management requires addressing several challenges:

  • Volume: A medium-sized enterprise may generate billions of log entries per day. Without automated collection and analysis, critical events are lost in noise.
  • Integrity: Attackers who compromise a system frequently attempt to delete or modify logs to cover their tracks. Logs should be forwarded to a centralized, hardened log server in real time. Write-once storage and cryptographic chaining (similar to blockchain principles) can ensure tamper evidence.
  • Retention: Regulatory requirements and forensic needs dictate how long logs must be kept. PCI DSS requires at least one year of audit trail history, with a minimum of three months immediately available for analysis.
  • Normalization: Different systems produce logs in different formats. Normalizing logs into a common schema enables correlation across sources.
  • Time synchronization: All systems must use a common time source (e.g., NTP — Network Time Protocol) so that events can be correlated chronologically across systems.

4.3 SIEM Systems

Security Information and Event Management (SIEM) — A technology solution that aggregates and analyzes log data from across an organization's infrastructure, applies correlation rules and analytics to detect threats, and generates alerts for security analysts. SIEMs combine the functions of security information management (SIM) and security event management (SEM).

Modern SIEM platforms (such as Splunk, IBM QRadar, Microsoft Sentinel, and Elastic Security) perform several functions:

  1. Log aggregation: Collecting logs from firewalls, servers, endpoints, cloud services, and applications into a centralized repository.
  2. Normalization and parsing: Converting diverse log formats into a unified schema.
  3. Correlation: Identifying patterns across multiple log sources that indicate malicious activity. For example, correlating a failed VPN login from a foreign IP address with a successful login five minutes later from the same IP might indicate a brute-force attack that eventually succeeded.
  4. Alerting: Generating prioritized alerts based on predefined rules and machine learning models.
  5. Dashboards and reporting: Providing real-time visibility into the security posture and compliance status.
  6. Forensic investigation: Enabling analysts to search historical data to reconstruct the timeline of an incident.

The effectiveness of a SIEM depends critically on the quality of its detection rules and the skill of the analysts interpreting alerts. An uncalibrated SIEM overwhelms analysts with false positives — a phenomenon called alert fatigue — while an under-configured SIEM misses real threats.

4.4 Cybersecurity Audit Programs

A cybersecurity audit systematically evaluates an organization’s security controls, policies, and practices against established criteria. Audits may be internal (conducted by the organization’s own audit team) or external (conducted by independent auditors).

ISACA and Cybersecurity Auditing

ISACA (originally the Information Systems Audit and Control Association) provides globally recognized frameworks and certifications for IT auditing. The CISA (Certified Information Systems Auditor) designation is a benchmark credential for audit professionals. ISACA’s approach emphasizes:

  • Risk-based auditing: Focusing audit effort on areas of greatest risk rather than attempting to examine everything.
  • Control objectives: Defining what each control should achieve (using frameworks like COBIT) before testing whether it does.
  • Evidence gathering: Collecting sufficient, reliable, relevant, and useful evidence to support audit findings.
  • Reporting: Communicating findings with appropriate context, materiality assessments, and remediation recommendations.

Forensic Readiness

Forensic Readiness — An organization's ability to maximize its use of digital evidence while minimizing the cost of investigation. This involves proactive planning for evidence collection, preservation, and analysis before an incident occurs.

Forensic readiness requires that logging be sufficiently detailed and that evidence be preserved in a legally admissible manner — maintaining chain of custody, using write-blockers for disk imaging, and following established forensic procedures. Organizations that invest in forensic readiness before an incident occurs are dramatically better positioned to respond effectively when one happens.


Chapter 5: Cyber Risk and Compliance

5.1 Understanding Cyber Risk

Every organization faces cyber risk — the potential for financial loss, operational disruption, reputational damage, or legal liability arising from failures in information technology or cybersecurity. Risk management is the disciplined process of identifying, assessing, and treating these risks.

Cyber Risk — The potential for loss or harm related to technical infrastructure, use of technology, or reputation, arising from a cybersecurity event. Cyber risk encompasses the probability of occurrence, the vulnerability exploited, and the impact to the organization.

5.2 Risk Assessment Methodologies

Qualitative Risk Assessment

Qualitative assessment uses descriptive scales (e.g., Low/Medium/High/Critical) to categorize the likelihood and impact of risks. A risk matrix plots these two dimensions to produce a risk rating.

Qualitative assessment is fast, intuitive, and useful when precise data is unavailable. Its weakness is subjectivity — two assessors may rate the same risk differently.

Quantitative Risk Assessment

Quantitative assessment assigns monetary values to risk components:

  • Asset Value (AV): The value of the asset being protected.
  • Exposure Factor (EF): The percentage of the asset value lost if the threat materializes.
  • Single Loss Expectancy (SLE): \( SLE = AV \times EF \)
  • Annual Rate of Occurrence (ARO): How many times per year the threat is expected to materialize.
  • Annualized Loss Expectancy (ALE): \( ALE = SLE \times ARO \)
Example: A company's e-commerce platform generates $10 million in annual revenue (AV = $10,000,000). A DDoS attack is estimated to cause 5% revenue loss (EF = 0.05), giving SLE = $500,000. If DDoS attacks occur twice per year (ARO = 2), then ALE = $1,000,000. If a DDoS mitigation service costs $300,000/year, the investment is justified because it is less than the expected annual loss.

Hybrid Approaches

Most organizations use a combination: qualitative methods for initial screening and prioritization, followed by quantitative analysis for the highest-priority risks where sufficient data exists.

5.3 Risk Treatment

Once risks are assessed, organizations must decide how to handle each one. There are four fundamental treatment strategies.

Risk acceptance must be a conscious, documented decision by management, not a default resulting from ignorance. Accepted risks should be reviewed periodically as conditions change.

Institutional Risk Management Failures

The consequences of poor risk management can be devastating. Consider the case of a large university that suffered a cybersecurity crisis when attackers exploited weaknesses in its IT infrastructure. Despite repeated warnings from internal security staff about unpatched systems and insufficient access controls, institutional leadership delayed investment in remediation, treating cybersecurity spending as a cost centre rather than a strategic necessity. 10.3 Third-Party and Supply Chain Risk Management

Modern organizations depend on complex ecosystems of vendors, cloud providers, open-source software projects, and business partners — each of which can introduce cybersecurity risk.

The SolarWinds attack (discovered in December 2020) is the defining case study in supply chain risk. 10.4 Cyber Insurance

The cyber insurance market has matured significantly but remains challenging. Chapter 11: Current Trends and Emerging Challenges

11.1 Zero Trust Architecture

Zero Trust — A cybersecurity strategy based on the principle of "never trust, always verify." Zero trust eliminates implicit trust based on network location and instead requires continuous verification of every user, device, and network flow, regardless of whether they are inside or outside the corporate perimeter.

Traditional network security followed a “castle and moat” model: once inside the network perimeter (the moat), users were implicitly trusted. This model fails in a world of cloud computing, remote work, and sophisticated lateral movement techniques. The 2020 SolarWinds attack demonstrated that attackers inside the perimeter could move freely across trusted network segments.

Zero trust architecture is built on several pillars:

  2. Least privilege access: Grant only the minimum access necessary, using just-in-time (JIT) and just-enough-access (JEA) principles.

OT systems (including SCADA and industrial control systems) were historically air-gapped from IT networks, but convergence has connected them to the internet, exposing them to cyber threats. The 2015 and 2016 attacks on Ukraine’s power grid — which caused physical blackouts affecting hundreds of thousands of people — demonstrated the real-world consequences of OT compromise.

11.4 Quantum Computing and Cryptography

Quantum computers exploit quantum mechanical phenomena to solve certain computational problems exponentially faster than classical computers. This capability threatens the mathematical foundations of much of today’s cryptography.

The Quantum Threat

  • RSA and Elliptic Curve Cryptography: Both rely on the computational difficulty of factoring large numbers (RSA) or computing discrete logarithms on elliptic curves (ECC).   • Supply chain attacks: Following SolarWinds, supply chain compromise has become a favoured technique for sophisticated adversaries, targeting the trusted relationships between organizations and their software vendors.
  • AI-augmented attacks: As discussed in Chapter 8, AI is enabling more convincing social engineering, faster vulnerability discovery, and automated attack tool development.
  • Geopolitical cyber operations: Nation-state cyber activities — espionage, sabotage, influence operations — are an established dimension of international conflict, as demonstrated by Russian cyber operations against Ukraine and Chinese cyber espionage campaigns targeting intellectual property.
  • Regulatory acceleration: Governments worldwide are imposing stricter cybersecurity requirements, mandatory incident reporting, and greater personal liability for security executives.
